Applications are taken by appointment only, at the civil status counter. The online pre-application on the website of the national agency for secure documents (ANTS) shortens your visit.
The document must be collected from the town hall within three months of being made available.

- Identity photo less than six months old
- Proof of address
- Previous document or birth certificate
